Academic Year 2015 Freshmen Health Exam Explanation and Notices

  • 1. According to the Regulations for the Implementation of Health Examinations for First-year Students, students must submit a health exam report when they enroll for the first year. (First-year student refers to students entering an undergraduate, masters or doctoral program or continuing education program, including students admitted into a graduate program directly from an undergraduate program of the same college.)
  • 2. Time of health exam: College freshmen should receive their health exam at the arranged time (Attachment); first-year students of masters, doctoral and continuing education programs should receive their health exam as follows (students of on-the-job programs may choose any one of the following time periods).
    (1) Students of the College of Arts, College of Technology, College of Science, and College of Extension Education should receive the health exam at 1:00~4:00 P.M. on September 17th, 2015 (Thursday)
    (2) Students of the College of Social Science and Physical Education, College of Management, College of Engineering, College of Education, and College of Extension Education should receive the health exam at 1:00~4:00 P.M. on September 18th, 2015 (Friday)
  • 3. Place of health exam: B1 of the Activity Center in Jinde Campus (handicapped students should call the Health Services Office on the day of the health exam, so that the office can send someone to provide assistance during the health exam).
  • 4. Please bring the health exam fee of NT$725; students with a low income household certificate are exempted.
  • 5. Please fill out the “National Changhua University of Education Student health card” (the part for students or parents to fill out), and bring the card to the site of the health exam on the day of the health exam. Please do not send the card back in advance.
  • 6. Fasting is not necessary before the health exam, and students with chronic diseases, e.g. high blood pressure, diabetes, etc., should not stop taking their medication.
  • 7. Notices for the day of the health exam:
    (1) Wear light clothes and do not wear any metal accessories or carry precious objects.
    (2) Female students should notify the nurse if they are on their period.
    (3) Female students should avoid taking the x-ray if they are pregnant or could possibly be pregnant.
  • 8. Notices for individual health exams (for students who cannot be present on the day of the health exam):
    (1) Students that received a health exam between April and September, 2015 should verify if the items of the exam match the items of NCUE’s health exam, and should receive exams for any missing items. After the hospital seal is affixed, copy the results and fill out the “National Changhua University of Education Student Health Card” (the part for students or parents to fill out), and mail or personally deliver the card to the Health Services office before September 26th, 2015.
    (2) Students may receive their health exam at the contracted hospital of NCUE – “Physical Examination Department of Changhua Show Chwan Memorial Hospital” or other hospital between August 1st and September 30th, 2015. Items of the health exam must be the same as the student health card. Students must make a copy of the results and mail or personally deliver the card to the Health Services office before September 26th, 2015.

  • 10. If students who are 20 years or older and do not agree to send results of their health exam to their parents or guardian, students should pay the fee on the day of the exam, and fill out “Do not send freshmen health exam results to parents”. The health exam results will then only be sent to the student.
